Job Summary
As a key member of the Behavioural Support Team, the RPN will work as a part of a team in providing support to staff caring for patients across all inpatient areas who are experiencing responsive behaviours. The team will conduct assessments and identify additional resources or supports required to support staff and patients. They will communicate a care plan, including recommended ongoing interventions, and schedule regular follow up. The Behavioural Support RPN will work with other team members to observe and assess patients, and participate in the development of care plans while collaborating with inpatient unit nurses, allied team members, and LOFT. The Behavioural Support RN will also help prepare for and participate in Behavioural Rounds, tertiary care consultations, work with discharge planning teams, and liaise with psychiatric consult liaison nurse when required. The Registered Practical Nurse (RPN) role models clinical excellence and the core competencies of the practice standards of the College of Nurses of Ontario (CNO), as well as the policies and procedures of Southlake Regional Health Centre (Southlake). The RPN works collaboratively as a member of the health care team. The RPN provides direct patient care to patients based on the three-factor framework as outlined in the College of Nurses of Ontario standard, RN and RPN Practice: The Client, Nurse and the Environment. These three factors have an impact on decision making related to care-provider assignment as well as the need for consultation and collaboration among care providers. All nurses make a commitment to lifelong learning.
Skills & Abilities
Recent related experience in acute general medicine nursing preferred
- Recent experience with initiation and maintenance of IV therapy preferred
- Ability to set priorities and organize work in response to patient/client needs
- Able to work 12 hour shifts, nights, and weekends
- Ability to function to the full scope of nursing practice as per the College of Nurses of Ontario
- Demonstrated commitment to ongoing learning, professional development, and continuous quality improvement
- Demonstrates understanding and complies with patient and staff safety policies and procedures that foster a safe and inclusive environment for all
- Experience with health teaching preferred
- Experience with accessing, maintaining and blood sampling from central venous access devices (CVAD)
- Knowledge of the complex and geriatric medical populations
- Demonstrated knowledge of skin assessment and wound care
- Good working knowledge of Electronic Documentation
- Demonstrated commitment to patient and family centred care and teamwork
- Demonstrated excellence in communication, both written and verbal
- Demonstrated problem assessment skills
- Demonstrated commitment to customer service and teamwork
- Demonstrated individual leadership skills

Qualifications
RPN diploma from a recognized school of nursing RPN Certificate of Registration, in good standing, from the College of Nurses of Ontario Current BLS certification required Gentle Persuasive Approaches (GPA) course preferred Physical Assessment course preferred
